Kuuku Dadzie's side need a good result from the second leg in Yaounde to reach the second round ahead the Championship in Equatorial Guinea, later in November.
Both sides finished the opening half goalless after a series of misses in front of goal.
After the interval, the Indomitable Lionesses struck first in the 73rd minute of the first round qualifier as they led the tie.
However, the Black Queens, who missed many chances finally found the back of the net with 13 minutes remaining.
Midfielder Florence Okoe scored the equaliser to hand Ghana a share of the spoils and set up a decisive return clash in a fortnight.
