In 2021, the final was played at the Madina AstroTurf, where Skyy FC faced Tema Youth in a thrilling encounter. The two sides battled to a 1-1 draw, with Ebenezer Mamatah scoring for Skyy FC and Emmanuel Owusu finding the net for Tema Youth. With nothing separating the teams after regulation time, the contest went to penalties. Skyy FC held their nerve to win the shootout 7-6, securing a dramatic triumph.

A year later, the action moved to the Kyebi AstroTurf for the 2022 final, where Liberty Professionals came up against Nsoatreman FC. Unlike the previous year's drama, this final was decided by a single goal. Seedorf Asante scored the decisive goal as Liberty Professionals claimed a hard-fought 1-0 victory.

The 2023 final, held at the Dr. Kwame Kyei Sports Complex, once again produced a narrow result. Bofoakwa Tano faced Skyy FC, and it was Saaka Dauda who made the difference. His goal handed Bofoakwa Tano a 1-0 victory and the coveted play-off final triumph.

In 2024, the drama returned to the penalty spot at Nana Kronamansah Park in Nsoatre. Vision FC and Okwawu United could not find a breakthrough during the match, finishing 0-0 after a tense contest. The final was eventually decided by penalties, where Vision FC prevailed 7-6 to claim victory in one of the most nerve-racking finals in recent years.

Then came 2026, with the final taking place at the Obuasi Len Clay Sports Stadium. Ebusua Dwarfs and Semper Fi produced another closely fought encounter, ending 1-1 in regulation time. Once again, the penalty shootout had the final say. Ebusua Dwarfs kept their composure to win 5-4 on penalties, adding another dramatic chapter to the history of the Division One play-off finals.

From Skyy FC's shootout victory in 2021 to Ebusua Dwarfs' triumph in 2026, the finals have consistently delivered tight contests, narrow victories and unforgettable penalty shootouts, highlighting the competitive nature of Ghana's Division One football.
